Client Intake Process
Step 1: Schedule A Intake Phone Appointment
Before scheduling an appointment in our office or online appointment through Zoom the counselor needs to have a brief 15 minute phone appointment with you. The reason for this call is to evaluate if your counseling need is something we can help you with and discuss our availability to meet your need.
To schedule this intake phone appointment, simply call our office at 317.585.1845 or use the intake form below.
The Administrative Assistant will help assist you in scheduling the intake phone call.
Step 2: Talk to the Counselor
At the scheduled time of your phone appointment you will call the office at 317.585.1845. Rev. Jeffrey Pokone will be expecting your call. During the phone appointment you will need to share a brief explanation of your counseling need. This will allow him to better assess your needs and determine how to assist you. Please feel free to ask any questions you might have during this call.
Step 3: Schedule Your Intitial Counseling appointment
Based on the counselor’s assessment of your needs and our availability to serve you he may suggest one of the following:
Schedule the initial in-office or online Zoom appointment.
We begin with an initial one hour counseling appointment before scheduling additional follow up appointments.
Recommend a resource or provide immediate counsel.
New clients must be prepared to wait 1-3 weeks from the time of their phone intake call to schedule their first appointment.
Online clients must submit their new client forms by email before their first intitial appointment.
Clients with office appointments must complete their new client forms and email them before their first appointment. If necessary you can print them and bring a hard copy to the office.
